Once it is filled out, sign it, then return it with payment to WEL using the directions on the application.

  • Criteria
  • Are or were a “C” Level Executive
  • Served as a member of executive management, within top two levels of organization (based on organizational structure)
  • Are or were a partner or shareholder in a professional services firm
  • Served as an active board member for a For Profit Company Or Nonprofit Organization (please identify the company/organization below)
  • Are or were a senior manager or officer at an organization (eg. Vice President, Assistant Treasurer)
